Report an Incident

From the Dashboard

From the Dashboard, in the Incidents section, Select Report an Incident

Continue to Report your Incident

From the Incidents Tab

From the Dashboard, Select Incidents

Select Report an Incident

Report your Incident

Enter Incident Info (see breakdown below) > Select Report Incident

  • Optionally, Select to Report the Issue anonymously
  • Enter the Date & Time of the Incident
  • Optionally, Assign an Investigator to the Incident
  • Enter a Title for the Issue
  • Assign the Issue a Category (options below)
    • Erroneous Release of Information by Fax, Phone or Email
    • Erroneous Release of Information by US Mail, FedEx, or other Courier services
    • Unauthorized Disclosure of PHI (either electronic or not) to an Unauthorized Party
    • Incidental Exposure of PHI in any other Manner
    • Other
  • Enter the Details of the Incident (guidance below)
    • When did you first discover the Incident?
    • When did you inform your Superior or Privacy Officer?
    • What actions did you and your Organization take following the discovery and investigation of this incident?
    • What remedial efforts are you taking to ensure this does not happen again?
  • Optionally, upload Supportive Documents, Images, or Videos

View Reported Incidents

You can always review past and active Incidents.

From the Dashboard, Select Incidents or in the Incidents section, Select See All

Here, you can see all of your Reported Incidents, review their Status in the Status column.

To View them, Select View in the Actions column

In the View Incident page, you can add Comments.

Add a Comment to an Incident

To add a Comment, Enter text in the Comment Text Box > Select Comment
